Based on the information that you provide, a decision will be made as to whether you need an assessment, and someone from an adult care management team will discuss the process with you.

Everyone is entitled to a free assessment if they are older, or frail, or have a disability and are having problems coping at home. Carers are entitled to a free assessment too. The council will work out risks to your independence with the aim of you staying in your own home for as long as possible. We can only support those needs that pose a critical or upper substantial risk to your safety or independence.
Anyone over the age of 18 who meets, or could meet, the Department of Health national fair access to care services eligibility criteria (FACS) is eligible for an assessment of their needs. The leaflet below gives information on what this may mean to you.

Eligibility Framework and East Riding of Yorkshire Council Threshold for Services
The Department of Health eligibility framework is graded into 4 bands, which describe the seriousness of the risk to independence and other consequences if needs are not addressed. The council will meet needs, that if unmet, will present risks that fall within the critical band. It will also meet needs that if unmet fall into the substantial band only if not meeting those needs would lead to Critical Band risks within 3 months. No other needs will be met.

Eligibility framework
If you have needs that are low, moderate or lower substantial they will not be met by the council but we will give you information and advice about other ways these needs could be met, for example through community-based services.
We can refer you to services that can help with problems like debt or housing, or advise on other short term issues. We can advise you where these services are or support you to contact them if you would like us to. Many of these services are free of charge but if not we cannot pay for them for you.

If some or all of your needs are eligible, we will work with you to agree what support is best for you, how much of that support if any will be funded by the council, and how you want to arrange that support package. You will be able to ask your care coordinator or assessment officer for advice along with any family member, friend or advocate you might have. We will also carry out a financial assessment at this time to see if you will need to pay a contribution to your care. The leaflet 'A Brief Guide to Charging' gives more information about this.

Introduction
You may have received a care service in the short term as an emergency, for example when leaving hospital. If you are likely to be in need of support longer term to remain safe and independent you will be offered an assessment of your needs. This will be done through what is called a “Supported Assessment Questionnaire”.
This is a simple questionnaire that is sent to you, with a covering letter, by care management. Your care management worker will arrange to meet with you and go through the Supported Assessment Questionnaire. You might want to have someone with you when the worker visits, family or friends for example.
The form covers things like how you manage with day to day tasks, how easy it is to get out and about in the community, how you keep yourself safe etc. You can fill it in on your own or with family and friends before the visit, or wait until the visit. Let us know what is working well for you and what you need support with. Shortly after the visit the worker will help decide whether or not you will be eligible for support and if so how much you will get – your Individual Budget.
The assessment
The assessment is about you, so you are the main person involved. It is often helpful if other people who know you well are also involved but this will only happen with your permission. This could include:
- your family;
- friends;
- carers;
- any health or social care professionals involved in your care and support.
You will be involved at every part of the process, and you will be involved in drawing up your personal support plan. Again this can be with other people such as your family/friends/carers if you choose.
The information you give is confidential and we will ask your permission before we share it with anyone, for example, doctors or occupational therapists.
Once the assessment is completed, this will result in a support plan, detailing your needs. Once this plan is agreed between yourself and the assessor, we will provide you with a copy, and we can also give a copy to your carer, or others involved in delivering your care, if you give your permission.
If your first language is not English or if you use sign language then we can arrange for a communicator. It would be helpful if you could identify this in your request so that arrangements can be made to provide the information in your preferred format.

When the assessment is finished the care management worker will discuss it with you and make sure you are content with what is in it – if you and your carer or the care management worker has different opinions in some areas this can be noted on the form. Sometimes the questionnaire can identify some things you might like assistance with, for example housing, or benefits or health and with your agreement the worker can pass your details on to someone who can provide specialist assistance.
When the questionnaire is finished to your satisfaction, it will be used to make a decision as to whether the needs you have identified are eligible for support (See Leaflet 5 on Fair Access to Care Services, which will tell you how this is decided).

Those needs that are eligible will be allocated a sum of money to them, through something called a Resource Allocation System, which calculates how much your Individual Budget will be. You will be informed in writing of your Individual Budget; you will be assessed to see if you have to pay a contribution to your support and how much this will be. If you do not agree with the Individual Budget you can tell your care management worker and their manager will look into this for you. Once the amount of the Individual Budget is agreed you will be able to go onto planning your support and further advice and information will be given to you.
